There are a total of 2 forms that you must complete and submit online for each backflow test. First you must complete the Request For Backflow Test Form. Then you must complete either the City of Tampa Backflow Assembly Test Report if within the city limits or the Hillsborough County Backflow Test Form if outside the city limits.
